To subscribe to The Healthspan Brief™ newsletter, you must provide a valid email address. By subscribing, you consent to receive periodic emails from Dr. Emmanuel Fombu related to the Healthspan Brief™, publications, events, and updates.

3. Intake Form for Concierge Service

By completing the Intake Form for the Concierge Service, you agree to provide accurate and truthful information. The information you provide will be used solely for the purpose of delivering personalized concierge services, including health assessments, diagnostic planning, and communication regarding your program.

You also consent to the secure storage and processing of your data, including health-related information, in accordance with our Privacy Policy.

4. Concierge Service Fiduciary Role and Payment Structure

By enrolling in the Concierge Service, the Client acknowledges and agrees to the following:

A. Fiduciary Role: Dr. Emmanuel Fombu acts in a fiduciary capacity with respect to the coordination and payment of third-party services included in the selected program tier. This means Dr. Fombu is committed to acting with a duty of care and duty of loyalty to the Client's best interests in securing and managing these services.

B. Payment Structure: The program fee paid by the Client is a bundled payment that is held in trust for two primary purposes:

  1. To cover the cost of the third-party diagnostic, clinical, and coordination services (e.g., Whole-Body MRI, DEXA/Coronary Calcium Scan, lab work, and network access) as outlined in the selected program tier.
  2. To cover the fee for Dr. Fombu's expert guidance, diagnostics interpretation, and personalized strategy.

C. Redistribution of Funds: The Client expressly agrees that a significant portion of the program fee is earmarked for redistribution to third-party vendors and service providers. Dr. Fombu's role as a concierge includes the responsibility to coordinate and pay these third-party costs on the Client's behalf. The funds allocated for these third-party services are not considered personal income to Dr. Fombu until they are disbursed to the respective vendors.

5. Intellectual Property

All content on this website, including but not limited to text, images, and publications, is the property of Dr. Emmanuel Fombu unless otherwise noted. Reproduction or distribution without written permission is prohibited.
